Company

Company overview

Most of Merck & Co.’s revenue comes from medicines that treat human diseases and vaccines that prevent them. It sells medicines to drug wholesalers, retailers, hospitals and government and managed-care buyers, rather than mainly to patients directly. Its business depends heavily on the cancer treatment Keytruda, its largest product. Vaccine brands include Gardasil and Gardasil 9; the U.S. government’s Vaccines for Children program buys a large part of its pediatric and adolescent vaccines.

Its smaller Animal Health business sells veterinary medicines, vaccines and health-management products and services for livestock and pets. Veterinarians, distributors, animal producers and pet owners are its buyers.

Merck’s earnings depend on patent protection and a continuing supply of successful new products and uses to replace sales lost as older products lose exclusivity. Insurers and governments negotiate prices and decide which medicines they cover, while U.S. laws require rebates and discounts for specified public programs and buyers. Merck must also obtain and maintain regulatory approval in each market before it can sell a product.

Merck’s results depend heavily on Keytruda, making Roche’s competing Tecentriq an important cancer-treatment comparison missing from this list.

  • Bristol Myers Squibb competes with Merck in cancer immunotherapy, but also receives a share of Merck’s Winrevair sales as royalties and pays Merck royalties on Reblozyl. Watch Reblozyl demand: its sales feed directly into Merck’s royalty revenue.

  • AstraZeneca is both a rival and a partner for Merck: its cancer medicine Imfinzi competes with Keytruda, while the companies share returns from Lynparza and collaborate on Koselugo. Lynparza sales are therefore relevant to Merck’s income as well as AstraZeneca’s.

  • Pfizer competes with Merck for sales of pneumococcal vaccines and COVID-19 medicines: its vaccines rival Vaxneuvance and Capvaxive, while Paxlovid competes with Lagevrio.

  • GSK competes with Merck for sales of preventive HPV vaccines, cancer immunotherapies and respiratory medicines. Its asthma and COPD inhaler business offers a comparison for Merck’s growing respiratory business, while its preventive HPV vaccines overlap with Merck’s vaccine business.

  • Sanofi and Merck compete in preventive vaccines, selling to publicly funded buyers among others, but jointly develop and distribute Vaxelis, a childhood vaccine covering six diseases. U.S. Vaxelis sales flow through their joint venture, so watch the partners’ share of its profits rather than their reported vaccine sales.

  • Johnson & Johnson competes with Merck in prescription medicines for pulmonary hypertension and cancer. Their former arrangement for Merck to market Remicade and Simponi has ended, so those products no longer provide the same business link.

  • Moderna shares an oncology trial stake with Merck through their jointly developed, individualized mRNA cancer therapy used with Keytruda. Results from that combination matter to both companies, unlike Moderna’s existing COVID-19 vaccine sales.

  • Regeneron competes with Merck in cancer immunotherapy through Libtayo, a rival to Keytruda. Libtayo news is the relevant comparison for Merck, rather than Regeneron’s EYLEA eye-medicine sales.

  • BeOne’s Tevimbra competes directly with Merck’s Keytruda in cancer immunotherapy, but BeOne’s largest drug business is Brukinsa. Watch Tevimbra specifically when assessing competition for Merck, rather than treating BeOne’s overall sales growth as a Keytruda signal.

  • Elanco competes with Merck Animal Health in products that protect pets from parasites, vaccines and livestock disease prevention. Its pet and farm-animal sales provide separate comparisons for Merck’s companion-animal and livestock businesses.

  • Gilead competes with Merck in HIV and COVID-19 medicines, but also jointly develops an HIV treatment combining islatravir and lenacapavir. News on that combination concerns a shared development project, not just competition for medicine sales.

  • Novartis competes with Merck for cancer-medicine sales, including treatments for advanced renal cell carcinoma alongside Merck’s Keytruda combinations and Welireg. Its everolimus-containing treatment is the concrete comparison to watch in that kidney-cancer market.
