Alto Neuroscience is a clinical-stage drug developer focused on conditions including depression, schizophrenia and Parkinson’s disease. Its most advanced candidate, ALTO-207, combines pramipexole and ondansetron to treat depression that has not responded to other treatments.
Alto uses its Precision Psychiatry Platform to analyze patient data, including brain-wave recordings and cognitive assessments, looking for biological markers that may identify patients likely to respond to a drug. For example, it is developing ALTO-300 for major depressive disorder in patients selected using a brain-wave marker. The FDA has not validated or affirmatively adopted Alto’s biomarker-based patient-selection approach; some candidates may also need separately approved tests to identify eligible patients.
Alto has no approved products or product-sales revenue and has funded operations mainly through equity financing and borrowing. It plans to sell drugs that receive regulatory approval, building its own commercial capabilities and exploring partnerships, especially outside the United States. Those sales would depend partly on government and insurer coverage and reimbursement; related diagnostic tests may need separate coverage.
The risk section is the longest, dullest part of the filing. This keeps only what can really hurt you: where it hits, how hard, when it plays out.
Every company claims an edge. Each one is checked against hard numbers like margins and returns: which hold, and which line gives way first.
Alto’s prospects hinge on clinical results rather than current product sales, especially for lead depression candidate ALTO-207, acquired from privately held Chase Therapeutics.
COMPASS Pathways Plc is developing COMP360 for the same treatment-resistant depression patients Alto targets with ALTO-207. Watch COMP360’s clinical progress; if approved, COMPASS plans to market it to U.S. healthcare providers and clinic networks.
Neumora Therapeutics, Inc. competes with Alto in developing drugs for major depressive disorder, so its depression trial progress matters more here than sales. Like Alto, it develops brain therapies in clinical trials; through December 31, 2025, Neumora had no approved products or product-sales revenue.
Xenon Pharmaceuticals Inc competes with Alto for patients with major depressive disorder and bipolar depression. Its investigational azetukalner is in Phase 3 trials for both conditions, making those results relevant to Alto’s ALTO-300 and ALTO-100 programs rather than a comparison of current drug sales.
Axsome Therapeutics, Inc already sells AUVELITY in the U.S. for major depressive disorder, where Alto is still developing antidepressants including ALTO-300 and ALTO-202. AUVELITY’s sales are the relevant line to watch, rather than Axsome’s combined revenue from depression, sleepiness and migraine medicines.
Helus Pharma Inc. competes with Alto in treatments added to existing therapy for major depressive disorder. Helus develops drugs designed to activate selected serotonin receptors, so progress in its adjunctive depression programs—not current product sales—is the news to watch alongside Alto’s ALTO-300.
LB Pharmaceuticals Inc is developing LB-102 for bipolar depression and as an addition to existing treatment for major depressive disorder, overlapping Alto’s ALTO-100 and ALTO-300. Watch its planned Phase 2 adjunctive depression trial, rather than treating LB-102’s Phase 3 schizophrenia development as depression progress.
NRX Pharmaceuticals, Inc. develops ketamine and combination therapies for severe depression and bipolar depression, overlapping Alto’s ALTO-207 and ALTO-100 programs. Watch its drug-development and FDA-submission news separately from the patient-service revenue it generates through Dura’s operations.
Neurocrine Biosciences Inc develops depression and schizophrenia therapies that overlap Alto’s psychiatric pipeline, but its current drug sales largely address other conditions. Its depression and schizophrenia development news is more relevant to Alto than sales of INGREZZA, which accounts for a significant share of Neurocrine’s product revenue.
Johnson & Johnson’s marketed Spravato treats treatment-resistant depression, giving Alto’s investigational ALTO-207 an established competitor to measure against. Spravato is the relevant product to watch, rather than Johnson & Johnson’s broader prescription-medicine business.
Minerva Neurosciences, Inc and Alto both develop treatments for unmet needs in schizophrenia, but target different problems: roluperidone addresses negative symptoms, while ALTO-101 addresses cognitive impairment. Watch roluperidone’s clinical and regulatory progress with that distinction in mind; Minerva has no approved products or product-sales revenue.
Axsome Therapeutics, Inc earningsAXSM
Axsome's AUVELITY sales will show how much US demand there is for a marketed depression treatment, a market where Alto is still developing its own medicines.
Focus on AUVELITY sales, not Axsome's combined revenue from other medicines. Stronger sales would show that patients, prescribers and payors are taking up a depression treatment in the US market Alto hopes to enter. Weaker sales would point to a harder commercial or coverage path, but would not tell us how Alto's biomarker-selected medicines will fare.
Xenon Pharmaceuticals Inc earningsXENE
Xenon's report will show how its azetukalner trials for major depressive disorder and bipolar depression are progressing in patient groups Alto also targets.
Watch for updates on Xenon's Phase 3 azetukalner trials in major depressive disorder and bipolar depression. Continued progress would put another candidate closer to evidence in patient groups targeted by Alto's ALTO-300 and ALTO-100, sharpening the competition; delays would ease that pressure but would not show that Alto's medicines work.
COMPASS Pathways Plc earningsCMPS
COMPASS's report will show how COMP360 is progressing for treatment-resistant depression, the same patient group Alto targets with ALTO-207.
Watch for COMPASS's updates on COMP360's clinical and regulatory progress. If the program advances, another potential treatment could move closer to the treatment-resistant depression patients Alto targets with ALTO-207, bringing more direct competition. If progress stalls, near-term pressure may ease, but that does not show ALTO-207 works or that COMPASS has abandoned its program.
Alto Neuroscience Inc. earningsThis company
Alto Neuroscience has no product sales yet, so its report will show whether its clinical programs are advancing and cash is still supporting development.
Read research and development spending alongside cash and clinical updates. Progress in the programs while cash still supports further development would show Alto turning its funding into clinical work. Spending without much visible progress, or a shorter cash runway, would make new financing or slower development more likely.
