Laboratory testing through BioReference is OPKO Health Inc.’s largest revenue source. Doctors and institutions order tests, and BioReference collects payment from insurers, Medicare and Medicaid, patients and institutional clients. After selling parts of the business, it retains clinical and women’s-health testing concentrated in New York and New Jersey, plus nationwide prostate-cancer and correctional-health testing. Its proprietary 4Kscore blood test helps assess the likelihood of aggressive prostate cancer before a biopsy decision.
OPKO also manufactures and sells medicines through operations in Ireland, Spain, Mexico and Chile. Its own Rayaldee treats secondary hyperparathyroidism in certain adults with chronic kidney disease and vitamin D insufficiency; it sells the drug in the United States through wholesalers supplying retail pharmacies and uses VFMCRP and Nicoya for international commercialization. OPKO developed NGENLA, a once-weekly growth-hormone injection marketed globally by Pfizer. Pfizer pays OPKO a share of gross profit under an arrangement covering both NGENLA and Pfizer’s Genotropin, so this income depends heavily on Pfizer’s commercialization efforts.
Healthcare coverage and payment rules shape how much OPKO collects, with government-subsidized programs supplying a significant part of revenue. BioReference must maintain federal laboratory certification and relevant state licenses to process patient samples. OPKO’s approved products can be promoted only for their approved uses, and international Rayaldee income depends on its regional partners’ development and marketing work.
The risk section is the longest, dullest part of the filing. This keeps only what can really hurt you: where it hits, how hard, when it plays out.
Every company claims an edge. Each one is checked against hard numbers like margins and returns: which hold, and which line gives way first.
Labcorp competes with OPKO’s remaining BioReference business for routine and specialty testing paid for by clients, patients and insurers, and also bought major BioReference operations. Watch Labcorp’s diagnostic-testing business against OPKO’s retained clinical and women’s-health testing in New York and New Jersey.
OPKO depends on Pfizer to sell its licensed growth hormone medicine NGENLA worldwide; Pfizer pays OPKO royalties and a share of gross profit from NGENLA and Genotropin. Those two products generated $31.9 million for OPKO in 2025, making their commercial performance the Pfizer results to watch.
Ascendis Pharma A/S competes for the growth-hormone-deficiency patients served by OPKO’s Pfizer-marketed NGENLA, selling long-acting growth hormone treatments. Its progress in that market matters to OPKO more directly than sales of its hypoparathyroidism medicine YORVIPATH.
MDxHealth SA’s Exo mdx competes with OPKO’s retained 4Kscore test by helping physicians and patients assess prostate cancer risk before a biopsy. Watch adoption of Exo mdx after MDxHealth acquired it in September 2025 and moved Select mdx customers to it.
Novo Nordisk sells long-acting growth hormone medicines that compete with OPKO’s NGENLA for growth-hormone-deficiency patients. For OPKO, watch those products within Novo Nordisk’s rare-disease business rather than its diabetes and obesity business.
OPKO shares ownership and development costs with Entera Bio Ltd. on specified programs that combine OPKO’s peptides with Entera’s N-Tab technology to turn injectable medicines into tablets. The oral long-acting PTH program is owned and funded equally; these programs are still in development, and Entera has no product-sales revenue.
Merck pays for rights to develop OPKO ModeX’s EBV vaccine candidate, so its development progress matters to OPKO’s licensing milestones. OPKO recorded a $12.5 million Merck milestone in 2024; watch news on that vaccine candidate rather than Merck’s overall medicine and vaccine sales.
Regeneron funds a collaboration with OPKO’s ModeX that combines Regeneron’s target-binding components with ModeX’s MSTAR platform to develop antibodies that engage multiple targets. OPKO received an upfront payment, making progress in this antibody collaboration the relevant news to watch.
Onconetix’s Proclarix blood test competes with OPKO’s 4Kscore when unclear PSA results prompt further assessment of prostate cancer risk. Proclarix is sold in Europe; watch its planned U.S. launch through a Labcorp license, which has not yet been described as an existing U.S. business.
Labcorp Holdings Inc. earningsLH
Labcorp’s report will show what testing demand and reimbursement look like for the routine and specialty services OPKO still provides in New York and New Jersey.
Look at testing activity and pricing in Labcorp’s diagnostics business, bearing in mind that its mix may not match OPKO’s retained tests. Stable or improving demand and reimbursement would point to a more supportive market for lab services; weakness would suggest greater revenue pressure for OPKO’s testing business, not prove that OPKO itself has declined.
Fed Interest Rate Decision
The Fed’s policy-rate decision signals the direction of borrowing costs; through the three-month SOFR-linked interest on OPKO’s 2044 Notes and its variable-rate credit facilities, it can change what OPKO pays to service debt.
Expected4.0%Last release4.0%
If it comes in above expectations
If policy rates rise more than expected, higher market rates can lift interest on OPKO’s SOFR-linked notes and variable-rate facilities, increasing the cash it needs for debt service.
If it comes in below expectations
If policy rates rise less than expected or fall, lower market rates can ease interest on OPKO’s SOFR-linked notes and variable-rate facilities, reducing the cash needed for debt service.
Pfizer Inc. earningsPFE
Pfizer’s report will show how NGENLA and Genotropin are doing commercially, the products whose gross profit Pfizer shares with OPKO.
Focus on Pfizer’s disclosures about NGENLA and Genotropin, not its overall results. Stronger sales or profitability for these growth-hormone products would support the income OPKO shares; weaker product performance would put that collaboration revenue under pressure.
Opko Health Inc. earningsThis company
OPKO Health’s report will show whether retained BioReference testing and pharmaceutical and collaboration revenue can sustain sales and narrow its operating loss.
Read diagnostic service revenue and pharmaceutical revenue separately, then compare them with the operating loss. Steady testing revenue, supported by product and collaboration revenue, alongside a narrower loss would show the retained businesses are strengthening the revenue base. Weaker service revenue or a wider loss would show that these streams are not yet covering operating and research costs.
Ascendis Pharma A/S earningsASND
Ascendis Pharma A’s report will show how its long-acting growth-hormone business is progressing, a competitive read-through for OPKO’s Pfizer-marketed NGENLA.
Look for Ascendis’s disclosures on sales or market progress for its long-acting growth-hormone treatment. Strong progress would suggest fiercer competition for growth-hormone-deficiency patients and more pressure on NGENLA’s opportunity; weak progress would offer less evidence of that pressure, but would not establish NGENLA’s own sales.
Entera Bio Ltd. earningsENTX
If Entera Bio reports progress on the oral long-acting PTH program, its report will show how OPKO’s jointly owned and equally funded research effort is advancing.
Look for concrete development updates on the oral long-acting PTH program. Clear progress would show that the jointly funded research is advancing; no substantive update would mean this shared investment has not yet demonstrated a development milestone.
