Company

Company overview

Drug developers need evidence that a treatment is safe and effective. Icon Plc helps design trials, organize physicians and research sites, recruit patients and manage studies worldwide. Its Accellacare research network supports patient recruitment. The company also provides related consulting and services after products reach the market. Pharmaceutical, biotechnology and medical-device companies pay for this work, alongside government and public-health organizations.

Clients generally agree to a fixed contract price, sometimes with variable components. They typically pay part of the fee when a trial starts and the rest in installments, sometimes tied to milestones. Contracts can run for several years, so revenue depends on studies starting and continuing, not simply on winning new work.

The business depends on clients continuing to fund and outsource research, and on recruiting suitable patients and investigators. Key customers supply a substantial share of revenue, but no single customer dominates. Clients can delay or cancel studies as results, funding or priorities change. ICON must also follow local clinical-trial rules and laws governing patient data; regulators can halt research, reject trial data or restrict data processing.

ICON depends on pharmaceutical and biotechnology sponsors’ clinical-development spending, with Parexel and Syneos Health also competing for that work despite being absent from this list.

  • IQVIA competes with ICON to run outsourced clinical-development programs for pharmaceutical and biotechnology companies. Its Research & Development Solutions business is the part to watch for competition over those customers’ spending.

  • Fortrea competes with ICON for drug sponsors’ spending on managing Phase I–IV trials, regulatory advice and post-approval evidence studies. Its Clinical Services revenue is a useful read on this overlapping work, with leading pharmaceutical customers accounting for 56% of its fiscal 2025 revenue.

  • Medpace and ICON both earn fees from pharmaceutical, biotechnology and medical-device companies for full-service Phase I–IV clinical development. Medpace’s work with small and mid-sized biopharmaceutical customers is worth watching for demand in a customer group ICON also serves.

  • Thermo Fisher’s PPD business competes directly with ICON to run large clinical-development programs for pharmaceutical and biotechnology customers. Watch PPD rather than Thermo Fisher’s overall results, which also include extensive laboratory-product businesses.

  • Charles River and ICON both depend on pharmaceutical and biotechnology customers’ drug-development budgets: Charles River handles drug discovery and non-clinical safety testing, while ICON runs clinical trials. Charles River’s Discovery and Safety Assessment revenue shows spending on the stages before clinical trials.
