Fortrea Holdings Inc. helps drug and medical-device developers run the studies needed for product development and regulatory approval. Customers pay it to manage clinical trials, study how drugs behave in people and advise on development and regulatory strategy. It can manage an entire trial program, supply experienced staff for selected tasks or combine both approaches. Many contracts use fixed prices or capped service fees.
Leading pharmaceutical customers supply most of its revenue, and one customer contributes a substantial share. The business depends on customers funding and outsourcing research, awarding contracts and continuing projects once work starts. Customers can delay, reduce or cancel work, so winning a project does not guarantee payment for its full planned scope. Delivering trials also requires suitable investigators and enough enrolled patients.
Fortrea must follow clinical-research standards and regulatory requirements governing its trials. Failure to comply can lead regulators to suspend studies or restrict operations.
The risk section is the longest, dullest part of the filing. This keeps only what can really hurt you: where it hits, how hard, when it plays out.
Every company claims an edge. Each one is checked against hard numbers like margins and returns: which hold, and which line gives way first.
Fortrea’s results hinge on sponsors’ clinical-development budgets and outsourcing decisions, with privately held Parexel and Syneos Health also important competitors beyond this list.
Icon Plc competes with Fortrea for drug and medical-device companies’ global Phase I–IV trials and outsourced clinical-development staff and services. Watch its large-sponsor business: ICON says large and medium-sized sponsors increasingly choose a limited number of providers.
IQVIA competes with Fortrea for large programs in which customers pay providers to run clinical development. Its Research & Development Solutions business is the relevant business to watch, rather than its healthcare software, data and commercial services.
Medpace Holdings, Inc. competes with Fortrea for biotechnology, pharmaceutical and medical-device customers paying to outsource Phase I–IV clinical development. Its full-service business with small and mid-sized biopharmaceutical companies is a useful place to watch competition for those customers.
Thermo Fisher Scientific’s PPD business competes directly with Fortrea to run large clinical-development programs for drug companies. Watch PPD rather than group-wide results: Thermo Fisher also sells a much broader range of scientific products and services to laboratories, hospitals and other customers.
Charles River Laboratories serves biopharmaceutical customers earlier in the same drug-development pipeline, providing outsourced discovery and nonclinical safety testing before the clinical trials Fortrea runs. Its Discovery and Safety Assessment business is the part to watch for research spending upstream of Fortrea.
Labcorp’s central-laboratory and early-development services draw on the same customers’ drug-development spending that funds Fortrea’s clinical trials. Watch those services in its BLS business, which supplied 22% of 2025 revenue; diagnostic testing accounted for the other 78%, making overall results a less direct guide to Fortrea.

Medpace’s report will show whether small and mid-sized biopharma customers are still paying for outsourced clinical-development work that Fortrea also seeks.
Look at demand for Medpace’s full-service work with small and mid-sized biopharma customers. Steady demand would show these customers still outsource clinical development, leaving Fortrea’s market for similar work intact; weaker demand would suggest those customers may be tightening research budgets, but would not establish Fortrea’s own new business.
Icon Plc earningsICLR
Icon Plc’s report will show whether large drugmakers are concentrating global trials and outsourced staff with a few providers, the same large programs Fortrea competes for.
Watch project demand in Icon Plc’s large-sponsor business, including the balance between new awards and ongoing work. If large customers place more work with a few providers, Fortrea faces more concentrated competition for big programs; if that work declines, the competitive pressure may ease, but it does not mean Fortrea won the business.

Fortrea’s report will show whether clinical pharmacology can offset weaker clinical-development and FSP work.
Read clinical pharmacology, clinical-development and FSP revenue together. Growth in the latter two as well would show that new work is broadening across Fortrea’s services; if growth still rests mainly on clinical pharmacology while the other two remain weak, the business is relying on a narrower mix and remains more exposed to study mix and pass-through costs.
