Company

Company overview

Humana arranges and pays for covered medical care for its health-plan members. Insurance supplies nearly all its revenue, led by individual Medicare Advantage plans, which provide Medicare coverage through a private insurer. The Centers for Medicare & Medicaid Services (CMS) usually pays Humana a fixed monthly amount per member, adjusted for demographics and health status. Members may also pay premiums and copayments.

Most revenue comes from federal-government contracts, making Humana heavily dependent on government payment rules and continued participation in those programs. Its insurance earnings depend on premiums covering medical care and operating costs. Premiums generally stay fixed for the contract year, so Humana cannot simply raise them midyear to recover unexpectedly high care costs.

Through CenterWell, Humana also provides pharmacy, primary care and home-health services, mainly to its own insurance customers but also to outside payers. Its senior-focused clinics operate chiefly as Conviva Senior Primary Care and CenterWell Senior Primary Care and largely earn payments under arrangements that share responsibility for care costs with health plans or CMS. Humana itself is not licensed to practice medicine; under certain management agreements, affiliated physician-owned groups retain control of medical decisions.

  • UnitedHealth competes with Humana for Medicare Advantage members, and both combine insurance with pharmacy services and care that puts providers at risk for patients’ medical costs. Watch Optum Health’s business of taking responsibility for care costs in exchange for monthly payments as a comparison for Humana’s CenterWell care business.

  • CVS Health’s Aetna competes directly with Humana for Medicare Advantage, prescription drug plan and Medicaid members; both also run pharmacy and primary care businesses. Aetna’s results in government-sponsored coverage offer a closer comparison for Humana than CVS’s retail pharmacy business.

  • Elevance Health competes with Humana in Medicare Advantage and pharmacy services, earning insurance premiums and pharmacy and care-service fees through Carelon. Its Medicare Advantage results and CarelonRx pharmacy business are the relevant comparisons, rather than its fees for administering employers’ self-funded health plans.

  • Centene competes with Humana for Medicare Advantage and stand-alone prescription drug plan members, as well as state Medicaid coverage contracts. Its focus on low-income members and its position as the largest stand-alone prescription drug plan provider make those drug plans and Medicaid contracts the businesses to watch for competitive pressure on Humana.

  • Alignment Healthcare competes with Humana for seniors choosing Medicare Advantage; both receive monthly payments from CMS and bear members’ medical-cost risk. Watch Alignment’s care costs against those payments and its membership growth for signs of pressure on Humana’s senior insurance business.

  • Clover Health competes with Humana in Medicare Advantage, and both use technology to support physician-led care and manage members’ medical costs. Clover pays doctors more for primary care visits using Clover Assistant; watch how its plans perform with that approach as a comparison for Humana’s data-supported care model.

  • Molina Healthcare competes with Humana for state Medicaid contracts and Medicare managed-care members, receiving premiums to arrange and pay for covered care. Medicaid supplied 75% of Molina’s consolidated premium revenue in 2025, making its state-contract news more relevant to Humana’s Medicaid business than to its Medicare-heavy business overall.

  • agilon health’s physician partners care for Humana Medicare Advantage members under agreements that pay a fixed amount per member each month, linking both businesses to that funding and patients’ medical costs. Watch agilon’s renewals and renegotiations with Humana: less favorable terms or terminated agreements can cost agilon patients and revenue.

  • Cigna’s Express Scripts competes with Humana’s businesses in managing prescription drug benefits for clients, a narrower overlap than Humana’s Medicare-heavy insurance business. Watch Express Scripts’ ability to secure the drug rebates it guarantees clients, rather than treating Cigna’s overall insurance results as a close comparison for Humana.

  • Humana pays Evolent Health to manage specialty care, with Evolent taking responsibility for medical claims in some arrangements and generally receiving a fixed monthly fee per member. Humana Insurance Company’s share of Evolent’s consolidated revenue fell from 19.3% in 2024 to below 10% in 2025, making Evolent’s overall results a less direct read on Humana.
