Company overview

X4 Pharmaceuticals centers its business on mavorixafor, sold as XOLREMDI, its only approved medicine. This once-daily oral treatment increases circulating mature white blood cells in patients with WHIM syndrome, a rare immune disorder. Its U.S. approval covers patients aged twelve and older. X4 makes it available through a specialty pharmacy but is not actively promoting it in the United States.

Revenue has come mainly from Norgine’s one-time license payment rather than drug sales. Norgine holds commercialization rights across Europe, Australia and New Zealand, subject to regulatory approval. It can also pay X4 milestones and royalties tied to future sales, while X4 supplies the drug under a cost-plus arrangement. International sales therefore depend on partners’ commercialization efforts, and patient access depends on insurers and government health programs covering treatment.

X4 is now prioritizing the 4WARD Phase 3 trial of mavorixafor for moderate and severe chronic neutropenia rather than investment in WHIM syndrome. That use is not yet approved. The business depends almost entirely on this one drug, and product sales have not been substantial enough to fund its operations; financing has primarily come from issuing securities and borrowing. X4 also relies on outside manufacturers, with Catalent its sole supplier of finished capsules.

X4’s prospects hinge on mavorixafor’s Phase 3 trial in chronic neutropenia and commercialization, with Norgine’s licensed markets offering milestone and royalty revenue.

  • X4 depends on Evotec SE to make the active drug ingredient in its lead medicine, mavorixafor, for both clinical trials and commercial supply. Evotec sells drug research, development and manufacturing services; for X4 holders, the supply of this ingredient is the part to watch.
