Guardian Pharmacy Services operates local pharmacies serving residents of U.S. long-term care facilities. Most revenue comes from residents of assisted living and behavioral health facilities; skilled nursing residents account for most of the rest. Its pharmacies fill physicians’ prescriptions, package medicines by resident and administration time, and deliver them to facilities. Medication tracking and caregiver training help facility staff manage residents’ medicines.
The facilities provide access to residents, but they are not always the payer. Most revenue comes through Medicare Part D prescription drug plans, under reimbursement agreements negotiated with those plans. Medicaid, commercial insurers and healthcare providers also pay Guardian. For residents whose skilled nursing stay is covered by Medicare Part A, the nursing facility pays Guardian under its contract rather than Guardian receiving payment directly from Medicare.
The business depends on residents choosing its pharmacies and on reimbursement terms. Preferred-provider agreements with assisted living and behavioral health facilities do not automatically secure residents’ business, and state freedom-of-choice rules can prevent facilities from requiring residents to use Guardian. Its pharmacies must maintain state licenses and meet government program requirements, including the service standards needed to participate in Medicare Part D’s long-term care pharmacy networks.
The risk section is the longest, dullest part of the filing. This keeps only what can really hurt you: where it hits, how hard, when it plays out.
Every company claims an edge. Each one is checked against hard numbers like margins and returns: which hold, and which line gives way first.
Guardian’s growth depends on winning long-term care residents against competitors including privately held PharMerica, while Medicare Part D reimbursement drives much of its revenue.
CVS Health’s Omnicare business competes with Guardian for the business of providing pharmacy services to long-term care facilities and their residents.
Guardian depends on Cardinal Health as one of its primary suppliers of drugs dispensed to long-term care residents; Cardinal earns revenue by selling drugs to pharmacies and other healthcare businesses. Its branded and generic drug supply and purchasing terms are the parts of its business most relevant to Guardian.
Guardian relies on McKesson as one of its primary wholesale suppliers for drugs dispensed to long-term care residents. McKesson sells branded and generic drugs to pharmacies and other customers through its North American Pharmaceutical business, making that business more relevant to Guardian than its prescription technology services.
Fed Interest Rate Decision
Guardian Pharmacy Services’ Fed-linked borrowing costs can rise or fall with interest rates if it draws on its revolving credit line, which was undrawn at year-end 2025.

If it comes in above expectations
Higher rates could make any future draw on Guardian’s credit line more expensive, increasing the cost of funding pharmacy operations or investment.
If it comes in below expectations
Lower rates could make any future draw on Guardian’s credit line cheaper, reducing the cost of funding pharmacy operations or investment.
CVS HEALTH CORPORATION earningsCVS
CVS Health’s report offers clues about its Omnicare long-term-care pharmacy business, which competes with Guardian Pharmacy Services for facility and resident customers.
If CVS reports stronger results tied to Omnicare, that would signal firmer competition for long-term-care pharmacy business and could make it harder for Guardian to win facilities and residents. Weaker results would soften that competitive signal, but CVS’s overall results may not isolate Omnicare’s performance.
Guardian Pharmacy Services, Inc. earningsThis company
Guardian Pharmacy Services’ report will show whether serving more long-term-care residents and filling more prescriptions is also producing enough gross profit to cover drug costs and reimbursement pressure.
Read revenue growth alongside gross profit. If both rise, revenue from additional residents and prescriptions is translating into pharmacy profit; if revenue rises but gross profit lags, drug costs, reimbursement or business mix is squeezing what Guardian keeps from each dollar of sales.
